PhD students who are not citizens of a Member State of the European Union or a Member State of the European Free Trade Agreement (EFTA) - parties to the agreement on the European Economic Area who study in Poland and have a valid Pole's Card or have been recognized as persons of Polish origin within the meaning of repatriation regulations if are not subject to compulsory health insurance under another title, they can voluntarily insure themselves by concluding an appropriate agreement with the National Health Fund. In this situation, the health insurance contributions are declared by the Gdańsk University of Technology

To conclud health insurance, you have to:

1. Go to the Pomeranian Provincial Branch of the National Health Fund (NFZ) in Gdańskul. Podwale Staromiejskie 69, to sign a voluntary health insurance contract.

Required documents for the contract in the NFZ:

  • Certificate from the Doctoral School office on the status of PhD student,
  • Pole's card or document confirming Polish origin within the meaning of the provisions on repatriation,
  • Passport,
  • Document confirming registration in the Pomeranian Voivodeship
  • Social Security

2. With a Pole's Card / a document confirming Polish origin within the meaning of the provisions on repatriation and an agreement with the NFZ or a document that will be issued by the NFZ, one should report to the Doctoral School office in order to submit further formalities for health insurance application.

If the application at the Doctoral School's office was made on the basis of a document from the NFZthe contract with the NFZ should be delivered immediately after receiving it.

3. After completing the above-mentioned formalities, Gdańsk University of Technology will pay monthly health insurance contributions.

4. In order to receive free healthcare, you must complete the DECLARATION at any clinic that has a contract with the National Health Fund.
