PhD students who are not citizens of European Union Member States or a Member State of the European Free Trade Agreement (EFTA) - parties to the agreement on the European Economic Area and unable to prove a valid Pole's Card or a document certifying that they have been recognized as persons of Polish origin within the meaning of repatriation regulations, may conclude a voluntary health insurance contract with the National Health Fund and pay the monthly contribution yourself.

1. To conclud health insurance, Go to the Pomeranian Provincial Branch of the National Health Fund (NFZ) in Gdańsk, ul. Podwale Staromiejskie 69, to sign a voluntary health insurance contract.

Document needed for a contract in the NFZ:

  • Certificate from the Doctoral School office on the status of PhD student,
  • Passport,
  • Document confirming registration in the Pomeranian Voivodeship
  • Social Security

2. Register the contract in Social Insurance Institution – Zakład Ubezpieczeń Społecznych (ZUS) address: 3/8 Chlebnicka St (10 minutes walking distance from NFZ)

Required documents:

  • Passport
  • NFZ Agreement
  • registration form: ZUS ZZA (also available at ZUS)

Every month, a separate declaration called ZUS DRA must be submitted to ZUS. Nearest ZUS branch is at 9 Tuwima St. The deadline for submission of the declaration and payment is the 15th day of the following month (e.g. for January the ZUS DRA form must be filled in and paid by 15th of February). In case of change in the personal data the following documents: ZUS ZIPA and ZUS ZIUA have to be submitted within 7 days.

Payments: Bank account number will be given to you during the registration. Each person receives an individual bank account number. Currently monthly payment for Voluntary Health Insurance is PLN 59,66

HOW TO USE IT? You can visit a doctor who cooperates with NFZ, a health clinic or a hospital marked with NFZ logo. We recommend GUMed Health Centre, because there you will easily find a doctor who speaks English. Please make a sure to take the NFZ Agreement and last payment confirmation with you.
