Covering Polish/EU/EFTA doctoral students with "university" health insurance

We would like to remind you that Polish/EU/EFTA doctoral students are subject to compulsory health insurance, if they are not subject to such insurance on any other account (this is explained in the article on our website linked above), on the basis of the PhD student's statement and ZUS ZUA declaration submitted to the School Office during registration for the first year. Such doctoral students are registered for insurance by Gdańsk University of Technology. If they have the status of a family member of an insured person, they are not subject to this insurance by virtue of being a doctoral student.


If a doctoral student is covered by health insurance on other grounds (e.g. signing a contract of employment, a civil law contract on which contributions for this insurance are paid), a doctoral student is obliged to inform the School Office of this in each case, by submitting an Application for amending the PhD student's data provided in the statement of PhD student for the purpose of doctoral scholarship payment and insurance application, attaching to it appropriate declarations ZUS ZWUA (deregistration from health insurance - code 18 12 xx ) and ZUS ZUA (registration with a different code - 18 13 xx only for social insurance).


Such an application must be resubmitted to the Office whenever a doctoral student loses another title to health insurance (e.g. the contract is terminated, so that contributions are no longer paid on it) so that the Doctoral School can re-register the doctoral student for health insurance. The ZUS ZWUA (deregistration from insurance - code 18 13 xx) and ZUS ZUA (registration with a different code - 18 12 xx for all insurance) forms should be attached to the application again.


This is particularly important when taking up employment during the period of training at the Doctoral School, as a doctoral student who fails to do this may "drop out" of health insurance altogether, which will mean, for example, that they will not be able to access NHF (NFZ)  healthcare services or obtain an EHIC (EKUZ) card.

We would like to take this opportunity to point out that the doctoral student is also obliged to report any changes in data - personal and identification - by submitting the aforementioned application. They, too, are subject to reporting to Social Insurance Institution (ZUS), based on the appropriate forms.
