Documents to be filled in by candidates accepted for the first year
In order to obtain doctoral student status at our school, receive a scholarship and apply for insurance:
1) you need to arrive to Poland with a complete set of original hardcopies of your admissions documents confirming your previous education (diplomas), which initially have been uploaded to your account in the enrollment system as scan copies and now should bear a legalization/apostille stamp, if issued abroad;
2) you need to receive a formal administrative decision on admitting you to our doctoral school - only if documents provided in step 1 are OK, which means that formal requirements have been met;
3) you need to fill in the additional documents according to the list below, which can be downloaded from our website from the tab PhD Students -> Templates to download - only upon receiving the administrative decision mentioned in step 2 above. The documents should be filled in only after arriving in Poland and setting an account in a Polish bank.
Access to this tab is possible only after logging in to the site (use the same login data as for MojaPG). In case of a problem with access to this tab, please send an e-mail to helpdesk@pg.edu.pl with a screenshot with a copy to us.
Before you fill in the documents, you should read the instructions placed in the above tab and information concerning insurance published on our website in the tab PhD Students -> Formalities -> Insurance and contributions , in particular with reference to health insurance.
Due to the fact that the beginning of PhD training formally takes place on October 2, this date can be entered as the earliest date possible to be covered by ZUS insurance in ZUS forms. Please read the instructions for each form in the downloads.
The documents must be delivered in the original hard copy and bear your signature due to the requirements of Polish Social Insurance Institution (Zakład Ubezpieczeń Zdrowotnych - ZUS).
You won't be reimbursed for the period of your stay outside Poland - the starting date of your doctoral scholarship is the date of confirming your status of a PhD student at Gdańsk Tech.
Documents to be submitted:
- PhD student 's Oath
- Statement of PhD student for the purpose of doctoral scholarship payment and insurance application
- ZUS forms regarding registration for insurance (ZUS ZUA and if applicable also ZUS ZZA)
- Declarations: tax residence and employment
