[1st-4th-year PhD students] Announcement on medical examinations for PhD students of the Doctoral School | Gdańsk University of Technology

Dear PhD Students

On an ongoing basis, referrals for medical examinations will be added to your accounts in case the previous ones expire. You will be informed about this by email, and the referrals will be attached via e-Documents.

On the Doctoral School website under tab : PhD Students -> Formalities -> Downloads -> Templates -> For all doctoral students -> Part II, item 5, you will find updated documents related to mandatory periodic examinations for doctoral students:

• communication on medical examinations;
• referral template (for completion of risk factors by the supervisor);
• a list of harmful, arduous and hazardous factors from the Health and Safety Department.

For the whole process to be successful you need to:

• sign the e-Document with the referral, commit to undergo examinations 
• completion by the supervisor of the list of risks on the referral, at the same time enclosing the completed file in the e-Document with the supervisor's signature -> more information for supervisors can be found in the tab: PhD Supervisors - Information for PhD Supervisors (in Polish only);
• pickup of a referral for medical examination - preferably immediately after receiving the information from the Doctoral School Office.
