[2nd-4th year PhD students] Completion of the 2nd, 3rd and 4th year of PhD training at the Doctoral School

Report on research progress / PhD training in the Doctoral School

Certificate confirming participation in the seminar (doctoral/at the faculty/department) / reporting session

Questionnaire for assessing the progress of the PhD student's scientific work (not applicable to 3rd and 4th year)
Transferring, obtaining signatures of decision makers, and approving documents at the Doctoral School is conducted in the MojaPG system -> Student -> eDocuments.
The circulation of all the documents / eDocuments in the system (including the questionnaire) is initiated by the Doctoral school Office.

Template No. 3, filled in by the supervisor is available in the word format on the Polish version of the school's website in the tab: > Dla promotorów (for supervisors) (access after logging in). The PhD student provides the eDocument to his/her supervisor for the supervisor to attach the completed questionnaire.
